In order to use the Services, you will need to enter an email address and password, so that we may assign you a Login ID ( "ID "). After you create your ID, you will be able to use this ID to log in to the E-Psychiatry website and utilize the Services, including managing your account, searching for physicians, making appointments, and accessing any other services offered by E-Psychiatry .
When you are ready to utilize E-Psychiatry's Services, you will also be asked to create an Account or Profile. To create the Profile, we will need some personal information such as your name, address, telephone number, date of birth, e-mail address, gender, and other pertinent data that will be available for you to share with your Healthcare Provider.
The email address that you provide to E-Psychiatry will be used to validate your account as well as to provide you with messages from your Healthcare Provider, appointment reminders, or other pertinent health related information. E-Psychiatry will not share your email address with any other third parties and will not use it for advertising or sales purposes. E-Psychiatry may create a record of the services you receive, including video and/or audio files associated with all consultations, electronic medical records that may be uploaded or created as a result of treatment, and other medical test results. For certain services, such as for telemedicine health sessions and monitoring services we will collect credit card or billing/payment account information which we maintain in encrypted form on secure servers.
When using our Services, we will ask you for certain personal information, which may include information like your name, phone number, gender, occupation, credit card or other billing information, your email address and the email address of your contacts, home and business postal addresses and website URLs. We may also ask you to provide Personal Health Information (PHI) such as your weight, blood pressure, lab results, medications, health history, medications or other PHI to E-Psychiatry or any Healthcare Providers. You may also need to provide E-Psychiatry with your insurance information, including your insurance carrier and policy numbers, certain health information, including names and addresses of healthcare providers you have seen, your reason for scheduling an appointment with a Healthcare Provider, your medical history, and any other information or data that you provide when using the Services.
